What is a Health Coach?

A health coach provides one-on-one support to help you with self-management, medication adherence, and lifestyle goals.

Our Health Coaches come from your local communities and are trained to assist and educate you on managing chronic conditions. Our Health Coaches support people of all ages, including children.

Health Coaches work with individuals living with:

  • Diabetes (Type I and Type II)
  • Asthma
  • COPD (Chronic Obstructive Pulmonary Disease)
  • HTN (High blood pressure)
  • CHF (Congestive heart failure)

Whole-Person Support

Our coaches build trust and connection through shared experiences as part of the communities they serve. Health Coaches are trained to combine medical knowledge with compassionate engagement to help you understand your health, stay motivated, and develop lasting habits that support long-term well-being.

Clinical & Condition Expertise

Diverge Health Coaches receive training to help you understand your condition, manage your care, and reach your health goals. They learn about the five conditions we support—diabetes, asthma, COPD, high blood pressure, and heart failure—with a focus on self-management and lifestyle support.

Motivational Training

In addition to comprehensive condition training, our coaches are skilled in motivational interviewing—a person-centered approach to communication that empowers you to explore your motivations, set achievable goals, and sustain healthy behaviors.

Getting Started with Health Coaching

We’ll Call To Schedule Your First Visit: A member of the Diverge Health team will call to set up your first appointment. Doctors and insurance plans send us lists of people who may benefit from working with a health coach.

Meet Where You’re Comfortable: You can meet at home, your doctor’s office, the library, or another place that feels right for you. Virtual visits are also available.

Touch Base Weekly, Then Monthly: You’ll meet weekly for the first three months, then twice a month afterward—helping you stay consistent, encouraged, and motivated.

Take Small Steps That Add Up: Together, you’ll focus on simple, sustainable actions that lead to lasting improvements in how you feel.
