Adele had been participating in health coaching for several months when her Health Coach discovered her blood pressure was critically high at 225/128 during an in-home visit. She was experiencing symptoms, and the Nurse Practitioner advised immediate emergency care. Adele was hospitalized for two weeks with multiple serious complications, including diabetic ketoacidosis, stroke, and hypertensive crisis.
After discharge, she transitioned to a skilled nursing facility and later returned home. Throughout this time, the Diverge team stayed closely involved. Her Health Coach coordinated follow-up appointments, completed a post-discharge visit with the Nurse Practitioner attending virtually, and helped correct discharge documentation issues so she could receive proper home health services and safety equipment.
Today, Adele is consistently attending appointments with her primary care provider and specialists. She is using an insulin pump with continuous glucose monitoring, and her blood sugar and blood pressure are now controlled. With physical and occupational therapy in place, her mobility is improving, and she remains motivated and grateful for the support of her care team.
