Timothy was identified as a candidate for health coaching with a focus on high blood pressure and COPD. About a month into working together, Health Coach Korinna checked his blood pressure during a visit and found it dangerously elevated at 171/120. She immediately connected with the Nurse Practitioner, who reviewed his chart and discovered he had not picked up an important blood pressure medication in over 60 days.
Timothy mentioned that his medication bag had seemed lighter than usual, but he hadn’t questioned it. Korinna helped him call the pharmacy right away so the prescription could be filled the same day. By the next day, his blood pressure had already improved. Further review also showed he was not taking a prescribed blood thinner, and Korinna provided education on why that medication was critical in lowering his risk of stroke.
With continued coaching, Timothy has stayed engaged in follow-up care with his primary care provider and is better equipped to manage his medications. Early intervention and close coordination likely prevented a serious medical emergency.
